How to Inject Trimix: A Step-by-Step Guide for First-Time Users (With Dosing Tips)
Use this guide to review your Trimix injection routine. If you are injecting for the first time, complete your medical training with your prescribing physician or healthcare professional before using Trimix at home.
Trimix can feel intimidating at first. The process becomes much easier when you follow the same simple routine every time:
Prepare your supplies.
Confirm your prescribed dose.
Choose the correct injection area.
Inject carefully.
Watch the clock.
Know when to call for help.
This guide supports: not replaces: the instructions provided with your prescription.
Important: Trimix is prescription medication. Use only the formulation, dose, injection frequency, and emergency instructions provided by your licensed prescriber.
What is Trimix?
Trimix is an injectable erectile dysfunction treatment made with three medications:
Papaverine
Phentolamine
Alprostadil
These medications relax smooth muscle and increase blood flow in the penis. Unlike oral ED medications, Trimix is injected directly into the erectile tissue.
The exact strength can vary. One patient’s Trimix may be much stronger than another patient’s. Never copy someone else’s dose or use a generic online dosing chart.
Gather your supplies first
Set everything on a clean, well-lit surface before you begin.
You will typically need:
Your prescribed Trimix vial
A new sterile syringe
Alcohol swabs
Gauze or a clean alcohol pad
A proper sharps container

Wash your hands with soap and water. Check your medication before using it.
Do not use Trimix if:
The medication looks cloudy.
You see particles or floating material.
The vial appears damaged.
The medication is expired.
Your pharmacy gave you different storage instructions and you are unsure what to do.
Most Trimix formulations should be refrigerated and protected from light. Follow the storage directions from your pharmacy. You can also review our Trimix storage guide.
Step 1: Confirm your dose
Read your prescription carefully before drawing medication.
Check:
The prescribed amount
The syringe measurement
The medication strength
The maximum frequency
Any instructions about oral ED medications
Syringe markings can be confusing. Make sure you understand whether your instructions use units, milliliters, or another measurement. If you are unsure, stop and call your prescriber or pharmacist.
Do not increase the dose because the first injection feels slow.
Do not inject a second dose during the same occasion if the first injection does not work. The medication may still be absorbed, and another injection can increase your risk of priapism: a prolonged erection that may cause permanent damage.
Step 2: Draw Trimix into the syringe
Follow the exact preparation method taught by your healthcare professional.
The general process is:
Remove the protective cap from the vial if needed.
Wipe the rubber stopper with an alcohol swab.
Let the stopper dry.
Open a new syringe without touching the needle.
Draw air into the syringe according to your training instructions.
Insert the needle through the center of the rubber stopper.
Push the air into the vial.
Turn the vial upside down.
Pull the plunger back slowly.
Draw slightly more medication than your prescribed dose if instructed.
Tap the syringe gently to move bubbles upward.
Push the extra medication and air back into the vial.
Stop at the exact prescribed dose.
Do not force the plunger. If it is very difficult to push, the needle may not be positioned correctly during injection. Do not keep pushing against strong resistance.
Step 3: Choose the injection site
Trimix is injected into the corpora cavernosa: the erectile tissue along the sides of the penis.
Your clinician may teach you a specific location. Common instructions place the injection:
In the middle portion of the shaft
On the left or right side
At a 90-degree angle to the shaft
Away from visible veins
Some training guides describe the location using a clock-face position, such as 9 and 3 o’clock or nearby positions. Your clinician may recommend slightly different positions based on your anatomy and technique.
Avoid:
The top of the penis
The underside of the penis
The urethra area
Visible veins
Lumps, scars, plaques, sores, or irritated skin
The exact same spot each time
Alternate sides. Move slightly up or down within the safe area as directed by your prescriber. This helps reduce repeated trauma and possible scar tissue.

Step 4: Inject Trimix
Use a steady, controlled routine:
Sit or stand in a stable position.
Hold the head of the penis: not just the skin.
Gently pull the penis straight out so the skin is taut.
Clean the chosen injection site with an alcohol swab.
Let the skin dry.
Hold the syringe like a pen or dart.
Position the needle perpendicular to the shaft.
Insert the needle smoothly into the erectile tissue.
Slowly press the plunger.
Remove the needle in one straight motion.
You may feel mild pressure or brief discomfort. The plunger should not require extreme force.
If you feel sharp pain or strong resistance, stop. Remove the needle and contact your prescriber for guidance. Do not repeatedly probe the same area.
Step 5: Apply pressure and dispose of the syringe
Immediately after removing the needle:
Press gauze or an alcohol pad firmly against the injection site.
Hold pressure for at least 2 to 3 minutes.
Follow your clinician’s longer compression instructions if you take aspirin or a blood thinner.
Place the used syringe directly into a sharps container.
Do not reuse syringes. Do not share medication or needles. Keep used sharps away from children and pets.
A small spot of blood or mild bruising can happen. Contact your healthcare professional if bleeding continues, bruising is severe, or you notice increasing swelling, redness, warmth, discharge, or pain.
Trimix dosing tips for first-time users
The safest dosing advice is simple:
Start with the dose prescribed for you. Change it only with medical guidance.
Keep a brief injection record. Write down:
Date and time
Dose used
How quickly the erection started
Erection firmness
How long it lasted
Any pain, bruising, or other symptoms
This gives your prescriber useful information. It also helps prevent accidental dose increases.
Keep these rules in mind:
Do not inject more than once in 24 hours unless your prescriber specifically tells you otherwise.
Many protocols limit injections to no more than three times per week.
Do not take extra Trimix because the response seems delayed.
Do not combine Trimix with oral ED medication unless your prescriber approves it.
Do not use someone else’s vial, formula, or syringe.
Do not change from Trimix to Bimix or Quadmix without a new medical evaluation.
Trimix commonly begins working within several minutes, but timing varies. Follow your prescription instructions and allow enough time before deciding whether it worked.
Know the warning signs
A full erection that lasts too long can become a medical emergency.
Call your prescriber if:
The erection is too firm.
The erection lasts longer than expected.
You have repeated weak or failed injections.
You develop a new lump or curvature.
You have ongoing penile pain.
You have repeated bruising or bleeding.
Go to the emergency room immediately if a rigid erection approaches or reaches four hours. Do not wait for it to resolve on its own. Tell the emergency staff that you used Trimix and explain when you injected.
Do not rely on exercise, cold packs, or over-the-counter medication unless your own prescriber has given you a specific emergency plan. When it comes to priapism, fast medical care matters.
The University of California San Francisco patient guide and Memorial Sloan Kettering’s penile injection resource provide additional patient education on injection technique and prolonged erections.
Make the process easier
Your first injection may take extra time. That is normal.
Prepare the same way each time:
Use a clean surface.
Keep your supplies together.
Check the dose twice.
Inject on alternating sides.
Record the result.
Contact your care team with questions.
